Purpose:
Fast-track ambulatory abscess pathway (FAAP) is a standardised protocol of day-only Surgery implemented in Campbelltown Hospital since 2017. This FAAP was developed as a solution for the high volume of skin abscess presenting to the Emergency Department (ED) in a health system overloaded with emergency surgical cases and financial hardship. The aim of this study was to evaluate the success of FAAP protocol in the last five years.
Methodology:
A retrospective and descriptive study analysed the outcome of soft tissue abscess presentation to ED included into the FAAP between January 2019 and June 2023 in SSWLHD Campbeltown Hospital. The primary outcome being successful surgery within 48 hours of admission.
Results:
Of the 4353 cases registered as abscesses in the hospital emergency between January 2019 and June 2023, 2461 (57%) were designated to the General Surgical team. Of these, 2,277 (93%) entered the FAAP protocol and 184 (7%) were hospitalized as inpatient before surgery. Of the total FAAP cases, 97% were successfully operated on in less than 48 hours. Delay in the operating room turn-over was the main cause of protocol failure (51%), followed by the patient not presenting for surgery (16%) and as the third cause, the patient refusal of surgery (13%).
Conclusion:
FAAP is a program that has been proven to be successful for the management of abscesses presenting to ED over the last five years. The next step is the implementation of a new protocol to manage simple abscesses with local anaesthesia to save peri-operative costs.
