Purpose: Warrnambool Plastics and Reconstructive Services (WPRS) supplies a large regional and rural population with access to local dermatologists, plastic surgeons and referral to local oncology services. This allows patients earlier and easy access to surveillance, diagnosis, surgical and oncological treatment of Melanoma. This literature review aims to evaluate the benefits of having a comprehensive melanoma service available in a regional area and the correlation with melanoma breslow thickness at presentation. Methodology: A search of MEDLINE / PubMed, Embase, Trip and Cochrane Library was performed for all peer reviewed journal articles with key terms “Melanoma”, Breslow thickness” and “Regional and/or rural”. Results: Australia continues to have the highest incidence of melanoma worldwide with over 17,000 new cases in 2022. In regards to the diagnosis and prognosis of melanoma, breslow thickness is a well documented predictor of both disease free survival and overall survival rates, and early detection of thin Breslow thickness melanoma’s have high rates of cure. There have been some studies that indicate patients living in regional areas present with a later stage of melanoma compared with their urban counterparts, however the literature is rather limited. Conclusion: Based on the available literature, further detailed analysis into the Breslow thickness on presentation to our melanoma service is required, in order to determine the benefits of having similar services in regional and rural towns of Australia.
