Purpose: The primary aims of this pilot trial are to determine the feasibility and acceptability of incorporating a virtual multimodal program into the pre and postoperative periods for patients undergoing gastrointestinal cancer surgery. The secondary aim is to obtain pilot data on the likely difference in key outcomes, data elements and components that will guide future implementation studies and identify barriers and facilitators, to inform the development of a substantive randomised clinical trial. Methodology: This pilot Randomised Type I Hybrid Effectiveness-Implementation Trial will include adult patients scheduled to undergoing major gastrointestinal cancer surgery at Royal Prince Alfred Hospital or Chris O’Brien Lifehouse. Consenting patients will undergo baseline assessment and be randomised to a virtual multimodal hub (Intervention group) or usual care alone (Control group). The intervention will be delivered before (1-6 weeks) and after surgery (up to 3 months),involving usual care and exercise, nutritional, psychological and nursing interventions and group-delivered peer support. In addition, geriatrician will provide support to patients aged ≥75 years old. Results: A total of 20 patients undergoing gastrointestinal elective surgery with curative intent, including liver, pancreas, oesophagus, gastric and colorectal cancer resections will be recruited between February 2024 and July 2024. The trial is expected to complete patient follow-up by November 2024. Conclusion: This pilot trial will provide preliminary data that will inform the design and methodology of a future Randomised Type I Hybrid Effectiveness-Implementation trial investigating the effectiveness of pre and postoperative virtual multimodal program.
