Neoadjuvant chemotherapy in early stage triple negative and HER2+ breast cancer: a retrospective review

Data from the Australia and New Zealand Breast Quality Audit (BQA) has shown that patients with HER2+ and triple negative breast cancer (TNBC) referred for neoadjuvant chemotherapy (NAC) have been increasing steadily since 2016. The main purpose of this study is to determine the compliance rate of referral to NAC for patients with TNBC or HER2+ breast cancer in accordance with the BQA key performance indicator (KPI) and the rates of complete pathological response (pCR) in a single institution in Australia. The secondary outcome is to identify predictive markers of complete pathological response. A total number of 705 patients with invasive breast cancer were discussed in the breast MDT meeting and were enrolled in the study from January 2020 to May 2023. 154 patients with early stage HER2+ or TNBC were identified who fit the criteria for referral to NAC. 138 (90%) were discussed and considered for NAC. 101 patients went on to receive NAC with 38 (37.6%) achieving pCR. Data will be collected on patients’ characteristics including demographics, BMI, co-morbidities and medications. Disease and radiological characteristics; in particular tumour size, lympho-vascular space invasion, histological grade, Ki 67, breast density will also be explored. Finally chemotherapy characteristics such as the drug regime used and dose density will be investigated. This data will then be analysed to explore predictive factors for pCR. This study has shown that patients with early stage HER2+ and TNBC who are receiving NAC is increasing in Australia in keeping with current guidelines. 1. Duffield et al.
