Pyomyositis is a suppurative infection of skeletal muscle which is uncommon outside tropical settings and in the vast majority of cases is caused by gram positive bacteria. Gram negative pyomyositis is an even rarer entity, with only 28 cases of Klebsiella pyomyositis desccribed in the literature. We present a case of multifocal klebsiella pyomyositis in a 46 year old poorly-controlled diabetic. Initially involving the bilateral forearms, the patient subsequently developed klebsiella panopthalmitis, intra-muscular lower limb and prostate collections. This case highlights the difficulties of early diagnosis due to minimal superficial signs of infection. In patients where klebsiella is found to be the causative organism of pyomyositis, there should be a low threshold for investigating for “metastatic infection” to other sites.
