We describe here an easy, reproducible, 4-step method to mark an ALT free flap that requires 2 pieces of equipment readily available in any hospital. The total process to mark the ALT can be performed in under 30 seconds with great accuracy.
The materials required are any standard surgical marker and a new role of tape with the plastic container it comes with.
The figures demonstrate each step required to mark the ALT free flap. This can be used on any patient undergoing this procedure. It is efficient, effective, reliable and reproducible in all settings. It is also an easily teachable method to junior plastic surgery registrars/residents.

This method reduces the amount of equipment usually required by the surgeon to mark the ALT free flap down to 2 and reduces the number of steps from approximately 8-10 down to 4 simple steps.
