We present an unusual case on a delayed repair of the right infraorbital nerve in the context of a right open zygomaticomaxillary complex fracture from a motor bike accident. The patient presented to outpatient clinics for his facial bone trauma, concurrently with symptoms of pain and paraesthesia in the right infraorbital nerve distribution. Consequently, the nerve was explored in the operating theatre, with the findings of extensive scar tissue and a completely divided infraorbital nerve. Neurolysis and repair of the nerve were performed using the medial cutaneous nerve of forearm as a cable graft. Two months post operatively the patient reported some return of sensation and improvement in pain. This case is an interesting and unique example of a delayed infraorbital nerve repair using a cable graft with noticeable clinical improvement several months post operatively.
