An approach to managing Extramammary Paget’s Disease

PURPOSE: Extramammary Paget’s disease (EMPD) is a rare cutaneous neoplasm that typically affects regions where apocrine glands are found, such as genitals and perineum. It presents as an indolent erythematous plaque which can mimic many other common conditions including psoriasis or eczema, making diagnosis challenging. Although EMPD is primarily confined to the skin, it can metastasise and can also occur with synchronous malignancies including colorectal, bladder, prostate, and cervical carcinomas. Given the diverse presentations, there are no widely accepted guidelines for the work up and management of EMPD. Our complex skin multidisciplinary meeting (MDM) at a quaternary cancer centre includes plastic surgeons, dermatologists, pathologists, medical and radiation oncologists. Through this MDM, we propose an algorithm for the diagnosis and management of EMPD. METHOD: We present a series of patients diagnosed with EMPD. After a thorough history and examination, presentation at our MDM involves pathology review with further immunohistochemical staining to confirm diagnosis. Baseline investigations including CT/PET staging, chest x-ray, faecal occult blood test, and prostate specific antigen are then conducted. Patients are referred to the Urology, Colorectal and Gynaecology (if required) departments to assess the need for further investigation. Furthermore, the MDM acts to formulate a management plan for patients which may include surgical excision, topical treatment, radiotherapy, or systemic therapy. CONCLUSION: We hope to bring this rare but important diagnosis to the attention of Australian practitioners and provide an approach to investigating and treating this condition.
