A ‘striking’ example of 2nd-5th CMCJ dislocation

Carpometacarpal joint (CMCJ) injuries constitute 0.2-1% of all hand injuries. Simultaneous dislocation of all four ulnar CMCJs is exceedingly rare, and the few cases reported in the literature are almost all from high velocity and high energy mechanisms. We present the case and impressive radiographs of a 21-year-old male who presented with closed dorsal dislocation of the 2nd-5th CMCJs, with concurrent hamate and 4th metacarpal shaft fractures, all from punching a tree. A review of the relevant anatomy and literature is also provided.
